Submit a completed claims-form in person, mail or email to:
- In Person: 27307 State Highway 189, Blue Jay, CA 92317
- Mail to: LACSD
- Attn: Human Resources & Risk Management
- PO Box 700, Lake Arrowhead, CA 92352
- Email to: HumanResources@lakearrowheadcsd.com
The claim will then be submitted to the Human Resources & Risk Management Department for processing.
